STATES & DISTRICTS / WISCONSIN / MILWAUKEE

Use this Milwaukee guide to find the correct crash-report request, understand local data coverage, and reach official traffic resources. Start with the investigating agency and report number on your paperwork.

Confirm Milwaukee Police completed the report

Milwaukee Police asks users to verify that MPD completed the crash report and that it is available. The department describes searches using a case number, name, or crash date and links to the appropriate report-purchase routes.

Its guidance distinguishes reportable crash reports, nonreportable forms, and supplemental investigative documents. A Wisconsin MV4000 copy does not automatically include audio, video, or other investigative material. Milwaukee County Sheriff and city MPD records are separate; check the investigating agency before ordering.

LOCAL RESOURCE CONTEXT

Traffic calming and crash records serve different purposes

Milwaukee’s Vision Zero resources describe a High Injury Network and traffic-safety work informed by a 2023 crash study. Community traffic-calming requests concern street design and prevention, rather than release of an individual police report. Keep the study date with any statistics and use the city’s reporting route separately for your own collision.

Wisconsin 511 provides regional travel information. Select the relevant roadway, direction, and event category and check the source’s own update time. Coverage may include construction, hazards, and incidents outside city limits; it is not a complete record of all local crashes.

Keep a useful record for follow-up

  • Save the investigating agency, report number, date, and precise road or intersection.
  • Retain your request confirmation and a dated copy of any report received.
  • Keep original photos, repair estimates, and towing or rental receipts together.
  • List missing documents or corrections for the records office separately from insurance questions.
